Vicious cycles of working harder, not smarter. More pressure leads to working harder and a short time fix of the time problem, but it triggers several reinforcing loops (some examples indicated in red) the would cause more and more problems.
Working harder helps, but its effect is decreasing. Reflections/communication is of increasing benefit. Unclear specification and unexpected incidents are the most severe threats followed by the unavailability of resources. Most important factor with increasing impact, however, is the motivation of the team.
